The city of Santa Bárbara de Samaná was founded in 1756 by the Francisco Rubio y Peñaranda. It was elevated to the status of municipality in 1865, to the Maritime District of Samaná.

In pre-Columbian times, the peninsula was officially recognized as Samaná by the Taino indians, which was written as "Xamaná" by the first chroniclers of the Indies. Santa Barbara was added in honor of the Queen Barbara de Braganza, wife of King Ferdinand VI of Spain, who ruled at the time of founding of the city.
